Health questionnaire

You should already have completed a health questionnaire.

This has been assessed by a senior nurse in the University’s Student Occupational Health Service, and will be used for two purposes: 

• to generate your certificate to commence the course;
• to generate your certificate for clinical placement that you will need to commence your hospital placement. 

If there is no problem with your health questionnaire, both certificates will be issued automatically to the pharmacy programmes office. 

If there are any issues, or further clarification is needed on whether you are up to date with your BCG vaccination, Occupational Health will either get in touch with you directly, or the Division will make an appointment on your behalf to attend Occupational Health during your first semester.
